Mayday and Rapid Intervention Teams: the culture of getting everyone out

A Mayday is a firefighter saying, out loud and on the radio, that they are in trouble. Everything else on this page exists to make that sentence easier to say and faster to answer: the training standard for the crew that comes to get them, the staffing rules that keep that crew standing by, and the culture that decides whether a firefighter calls for help while there is still time to send it.

Most of what determines whether a downed firefighter comes home is decided before the alarm. It is decided by whether the department trained a rescue crew, staffed one on scene, and built a culture in which calling a Mayday is a professional act rather than an admission of failure. This is a plain walk through what a Mayday is, why the hardest part is the decision to make the call, and what the national standards actually require of the crew waiting to answer it.

What a Mayday is

A Mayday is the radio transmission a firefighter makes when they are lost, trapped, disoriented, injured, running out of air, or otherwise unable to get out on their own. It is deliberately the same word aviation and the maritime service use for a life-threatening emergency, and it carries the same meaning: stop, this is about survival, everything now bends toward the person who called it.

The reason the fire service settled on a single, unmistakable word is that the situations behind it are the ones that kill firefighters most often. The International Association of Fire Fighters, describing its Fire Ground Survival program, states that firefighters "becoming trapped and disoriented represent the largest portion of structural fire ground fatalities." The Mayday is the first link in the chain that answers that problem, and a weak first link breaks the whole chain.

Because a firefighter in trouble is often low on air, disoriented, and speaking through a facepiece, many departments teach a fixed script so the essential information survives the stress. One widely taught version is the acronym LUNAR: location, unit, name, assignment or air, and resources needed. The point of a script is that the caller does not have to compose a sentence at the worst moment of their career; they only have to fill in the blanks.

The hardest part is deciding to call it

The technical parts of a Mayday can be taught in an afternoon. The part that fails is human. Firefighters hesitate, and the hesitation is what kills.

In his U.S. Fire Administration Executive Fire Officer research paper Mastering the Mayday, West Bend (Wisconsin) Fire Department author Gerald Kudek opens with the last radio transmission of Firefighter Kyle Wilson of Prince William County, Virginia: "MAYDAY, MAYDAY, MAYDAY, Tower 512 bucket, I'm trapped inside, I don't know where I am, I'm somewhere in the stairwell, I need someone to come get me out!" Wilson died in that stairwell on April 16, 2007. Kudek's paper documents a problem that is not unique to one department: personnel who train constantly on fire attack have "very limited experience or training in calling or handling a MAYDAY situation," and in his own department's records, across seventy-five actual fire calls in a year, "there were zero MAYDAY requests." A skill that is never rehearsed is a skill that is not there when the radio comes up.

Underneath the training gap is a cultural one. The instinct that gets rewarded on the fireground, self-reliance, pushing through, not wanting to pull a crew off the fight, is exactly the instinct that delays a Mayday. Firefighters try to self-rescue first. They worry about the embarrassment of calling for help and then walking out on their own. They do not want to be the reason the operation stops. Each of those is a reasonable feeling and each of them costs air and time that a trapped firefighter does not have.

This is where safety culture stops being an abstraction. A department whose culture treats a Mayday as a normal, expected, respected radio call, one that is drilled, debriefed without blame, and praised when it is made early, is a department where firefighters call it while there is still a rescue to perform. A department whose culture treats it as a failure teaches its people to wait, and waiting is measured in breaths. The willingness to key the radio early is not a personality trait. It is manufactured, or not, by the culture around the firefighter. That is the same argument that runs through what safety culture actually means on the fireground: the initiative that every firefighter must be empowered to stop unsafe practices includes the practice of dying quietly rather than calling for help.

The crew that answers: RIT and RIC

A Mayday is only as good as the crew ready to respond to it. That crew is the Rapid Intervention Team, also called the Rapid Intervention Crew or Company. The names and acronyms vary by region (RIT, RIC, and the older FAST, for firefighter assist and search team), but the function is fixed: a crew that does nothing else, standing by outside the hazard, fully equipped, whose only job is to go in and rescue a firefighter who cannot get out.

The distinction that matters is dedication. A RIT is not the next crew that happens to be free. It is assigned to rescue and held in reserve for it, so that when a Mayday comes there is no delay to assemble people, find air, and brief them. They are already dressed, already staged, already thinking about the building. Every minute saved at the moment of the call is a minute the trapped firefighter still has air.

What NFPA 1407 requires of that crew

Standing a crew by the door is not enough if they have never practiced the thing they are there to do. NFPA 1407, the Standard for Training Fire Service Rapid Intervention Crews, is the document that defines that training. According to coverage of the standard in Firehouse, NFPA 1407 "covers the training that's required to perform the much needed rapid-intervention team (RIT) task," setting out policy and procedures, instructor requirements, student prerequisites, the operations training program, and the performance standards a rapid intervention crew must meet. First published in 2010 and revised in 2015 and 2020, it is the reason a RIT is a trained assignment rather than a hopeful one.

NFPA 1407 also carries the standard definition of the crew itself: a Rapid Intervention Crew is "a dedicated crew of at least one officer and three members, positioned outside the IDLH, trained, and equipped as specified in NFPA 1407, who are assigned for rapid deployment to rescue lost or trapped members." IDLH is the fire service term for an atmosphere immediately dangerous to life or health, which is to say the inside of a working structure fire. As of the 2020 edition, NFPA has folded the standalone NFPA 1407 into the consolidated NFPA 1400, but the training requirements it established remain the reference point for RIT competency.

The rule that a rescue crew must be on scene

Two national documents make the rescue crew a requirement rather than a good idea. The first is a workplace-safety regulation; the second is the fire service's own consensus standard. They point the same direction.

The regulation is OSHA's respiratory-protection standard, and the specific provision is the one the fire service knows as "two-in/two-out." Under 29 CFR 1910.134(g)(4), for interior structural firefighting the employer must ensure that "at least two employees enter the IDLH atmosphere and remain in visual or voice contact with one another at all times" and that "at least two employees are located outside the IDLH atmosphere," with all firefighters using self-contained breathing apparatus. One of the two outside may hold an additional role, such as incident commander or safety officer, "so long as this individual is able to perform assistance or rescue activities without jeopardizing the safety or health of any firefighter working at the incident." The rule is the seed of the RIT: it says that from the moment firefighters go inside, there must be people outside whose availability to rescue them is protected.

OSHA's standard contains one deliberate exception, and it is worth stating plainly so it is not mistaken for a loophole. As OSHA has explained in interpreting the rule, if a known life is in immediate jeopardy and immediate action could prevent the loss of that life, initial firefighters may act before the full complement is assembled. It is a judgment for the incident commander at the scene, not a routine practice, and it exists so that the rule protecting firefighters does not forbid saving a civilian.

The consensus standard is NFPA 1500, the Standard on Fire Department Occupational Safety and Health Program, whose Section 8.8 is titled "Rapid Intervention for Rescue of Members." It requires that the fire department "provide personnel for the rescue of members operating at emergency incidents," and it builds the same two-tier structure OSHA implies. In the initial stages of a working structural fire with a single crew inside, NFPA 1500 requires "a minimum of four individuals," two working as a crew in the hazardous area and two standby members outside "available for assistance or rescue." Then it escalates: once a second crew is operating inside, "the incident shall no longer be considered in the 'initial stage,' and at least one rapid intervention crew shall be deployed." As the incident expands, that RIC becomes a dedicated on-scene crew whose only assignment is rescue, "fully equipped with protective clothing, protective equipment, SCBA, and any specialized rescue equipment that could be needed."

NFPA's deployment standard for career departments, NFPA 1710, describes the same progression in operational terms. It treats the first two firefighters staged outside as an initial rapid intervention crew drawn from the initial attack, and requires the incident commander to upgrade that initial crew to a full, dedicated rapid intervention crew of four as the incident grows in size, complexity, or risk. Read together, the three documents say one thing: a firefighter never operates inside a working fire without a crew positioned to come get them, and the size and dedication of that crew grow as the danger does.

Why this is a culture story and not a checklist

It would be easy to read the standards as a compliance exercise: staff four, dedicate a crew, log the training, done. The standards are necessary, but they are the floor, and the floor is not the same as getting everyone out.

A dedicated RIT can be standing by the door and still fail if the Mayday comes too late, because the culture taught the firefighter to wait. A department can meet two-in/two-out on paper and still lose people if the standby crew is treated as a formality to be raided the moment another task looks more urgent. And a RIT can be perfectly trained to NFPA 1407 and still be searching an empty quadrant if the caller could not get out a clear location under stress, because the department never drilled the radio report. Each of those failures is cultural before it is technical. The standards describe what a good system looks like from the outside. Whether the system actually works when a firefighter goes down depends on the thousand small decisions a department makes about what it trains, what it staffs, and what it honors.

The account in Kudek's paper of a Mayday that went right captures what the whole system is for. A disoriented crew "got together in the kitchen and called a MAYDAY," gave their location, and, in the firefighter's words, "almost immediately we heard the RIT coming up the porch stairs. They entered the building under extreme heat and zero visibility. We could hear them coming and yelled back and forth until we could see their hand lights. They escorted us out of the building." A crew willing to call early, a rescue team staged and trained to answer, and a location clear enough to find. That is the culture of getting everyone out, working exactly as designed.
